Is your child/ren on our CENSUS?
If you are new to the Blooming Prairie area and you have a/or child/ren please fill out the following form so your child/ren can be added to our district census.
The census is used to send out information about preschool and Early Childhood Screening. Please take the time to fill out as many forms as needed to let us know about your child/ren; so we can add them to our census.
The census we keep ranges in age from birth-5 years old. If your child is between those ages please fill out the form below.
